Health Outcomes & Disparities (1 to 2 pages): Discuss 1 or 2 negative health outcomes, health factors, or health behaviors that are disproportionately present in your target population. Compare the prevalence of each outcome in your target population to its prevalence in the general US population. Are there any historical factors we should be aware of that influence this outcome or factor? What are the consequences of the disparities – for example; a higher mortality rate, more children in poverty, the continuation of social injustice, higher cost to the healthcare system. Do not guess on this point, please use literature to support your ideas.
Social Determinants (1 page): Provide a literature review of at least 3 social determinants of health that are most closely associated with the occurrence of health disparities in this population that may relate to or influence the outcomes discussed above. . What is the mechanism i.e. diabetes is higher in this group due to lack of access to healthy food, low employment status, and high violent crime which makes outdoor exercise difficult?
